Edit: Got it working, played a bit of single player before bed.
Operation Guillotine, when you jump down into the canal and move around to the right, keep an eye on the guy you are following out of the canal.
He like stops by a rock and then randomly falls over dead, every single time. No bullets being shot at him or anything. I died like 7 times on that bit and I laughed every time I saw him fall over dead.
